The notorization on the 2nd page is only required if requested for special occasions. Forms expire 12 calendar months after the date of the doctor's and the participant's & guardian's signatures. Some parts of this form may be completed online before printing. High Adventure High Adventure Adult Leader Application Source: Mecklenburg County Council This form is to be completed and turned in to the Scout Service Center by Adult Scouters interested in applying to be a leader for a council contingent to a BSA high adventure base, such as Philmont Scout Ranch, Northern Tier Canoe Bases, Florida Sea Base, or a National Scout Jamboree. Further information about various oppotunities, schedules and application deadlines, and the interview and selection process should be obtained from he council Program Director or from the council camping or high adventure committee. Top Merit Badge Merit Badge Counselor Information Form Source: BSA Scouting.org Filestore This form is to be completed by the applicant and turned in to the Troop for submission to the Scout Service Center. The BSA Adult Registration Application must be attached only if the applicant is not already currently registered with BSA, either as as an Adult Leader or Merit badge Counselor. Merit Badge Counselors should also have completed and be current in BSA Youth Protection Training. OA Brotherhood Membership Requirements Guide Source: Troop 116 This information sheet will aid the Brotherhood candidate in pointing him to the appropriate references in the OA Handbook and will provide a ready reference as he learns the OA Obligation and Official Song. After obtaining required signatures it is to be filed with the Council Service Center two weeks in advance of the scheduled activity for proper clearance. If creating an account for the first time and you are already a BSA-registered adult, be sure to enter your BSA ID during initial setup. However, you do not have to be registered to create an account. Youth Protection Training - Troop 116 Member Status Source: Troop 116 List of Troop members showing date of Youth Protection Training Certification on file. Certifications are valid for 2 years. Current YPT training is required for all registered adults, all Merit Badge Counselors, and all other adults that participate in the troop program. Registration, registration renewal, and signup for a troop outing is not allowed unless YPT is current. Certification must be renewed every two years to stay current.
