Parts A, B, & C, plus photocopy of both sides of insurance ID, are required by Troop 116 and must be updated annually.
Forms expire 12 calendar months after the date of the doctor's and the participant's & guardian's signatures.

High Adventure Adult Leader Application

This form is to be completed and turned in to the Scout Service Center by Adult Scouters interested in applying to be a leader for a council contingent
to a BSA high adventure base, such as Philmont Scout Ranch, Northern Tier Canoe Bases, Florida Sea Base, or a National Scout Jamboree.
Further information about various oppotunities, schedules and application deadlines, and the interview and selection process should be obtained from
he council Program Director or from the council camping or high adventure committee. |

This form is to be completed by the applicant and turned in to the Troop for submission to the Scout Service Center.
The BSA Adult Registration Application must be attached only if the applicant is not already currently registered with BSA, either as as an Adult Leader or Merit badge Counselor.
Merit Badge Counselors should also have completed and be current in BSA Youth Protection Training. |

Youth Protection Training - Troop 116 Member Status

Certifications are valid for 2 years.
Current YPT training is required for all registered adults, all Merit Badge Counselors, and all other adults that participate in the troop program.
Registration, registration renewal, and signup for a troop outing is not allowed unless YPT is current. Certification must be renewed every two years to stay current. |
