| BSA Annual Health & Medical Record form Source: BSA Scouting.org Filestore Parts A, B, & C, plus photocopy of both sides of insurance ID, are required by Troop 116 and must be updated annually. Forms expire 12 calendar months after the date of the doctor's and the participant's & guardian's signatures. The Troop considers all camping trips that we schedule, plus other special events as defined, to require all parts of the form, A, B, and C. The Troop only accepts, logs, and files the complete forms. |

| High Adventure | |
| High Adventure Adult Leader Application Source: Mecklenburg County Council This form is to be completed and turned in to the Scout Service Center by Adult Scouters interested in applying to be a leader for a council contingent to a BSA high adventure base, such as Philmont Scout Ranch, Northern Tier Canoe Bases, Florida Sea Base, or a National Scout Jamboree. Further information about various oppotunities, schedules and application deadlines, and the interview and selection process should be obtained from the council Program Director or from the council camping or high adventure committee. |

| Merit Badge | |
| Merit Badge Counselor Application Source: BSA Scouting.org Filestore This form is to be completed by the applicant and turned in to the Troop for submission to the Scout Service Center. The BSA Adult Registration Application must be attached only if the applicant is not already currently registered with BSA, either as as an Adult Leader or Merit badge Counselor. Merit Badge Counselors should also have completed and be current in BSA Youth Protection Training. |

| Troop Resource Survey Source: BSA Scouting.org Filestore For adults to complete and turn in to the Troop leadership to indicate areas of interest, talents, and resources they are willing to use to help the Troop. |

| Tour Permit | |
| Tour Permit, Local Source: BSA For trips and camps under 500 miles. This application form may be completed online and printed. After obtaining required signatures it is to be filed with the local Scout Service Center two weeks in advance of the scheduled activity for proper clearance. |
|
| Tour Permit, National Source: BSA For tours 500 miles or more and tours outside the U.S.A. This application is to be submitted to the local Scout Service Center for approval at least one month before the tour. |
