Parts A, B, & C, plus photocopy of both sides of insurance ID, are required by Troop 116 and must be updated annually. Forms expire 12 calendar months after the date of the doctor's and the participant's & guardian's signatures.
The Troop considers all camping trips that we schedule, plus other special events as defined, to require all parts of the form, A, B, and C. The Troop only accepts, logs, and files the complete forms.

High Adventure

This form is to be completed and turned in to the Scout Service Center by Adult Scouters interested in applying to be a leader for a council contingent to a BSA high adventure base, such as Philmont Scout Ranch, Northern Tier Canoe Bases, Florida Sea Base, or a National Scout Jamboree. Further information about various oppotunities, schedules and application deadlines, and the interview and selection process should be obtained from the council Program Director or from the council camping or high adventure committee.
